ChiroFit (“we” or “our” or “us”) is committed to protecting your personal data and respecting your privacy.
This policy sets out the basis on which any personal data we collect from you from our website (www.chirofit.je) (“Website”), or that you provide to us by other means will be processed by us. For the purpose of the Data Protection (Jersey) Law 2018 (the “Law”), the data controller is ChiroFit.

(b) To stay in contact with you regarding matters such as appointments, payments, clinic updates, and promotional material;
(c) To exchange relevant information with your referring Medical Practitioner or Practice;
(d) To communicate with healthcare providers to whom we refer you;
(e) To liaise with your health insurance company;
(f) To share information with legal professionals and insurance brokers when required for insurance-related matters or personal injury claims.
Information will only be shared with your express permission or if there is imminent risk to yourself or others.
We may collect and process data about you, in accordance with the Law:
Some of this information is aggregated or encrypted and does not identify you personally but provides us with information about how you use and engage with the ChiroFit services and Website.
The data we collect may include, but is not limited to:
· Basic personal information, such as your full name, date of birth,
· residential address, phone number(s), and email address together with financial and credit card information
· Health-related and physical data, such as your medical history, any disabilities, current prescriptions, family medical history, your present medical condition
· Information that you provide by filling in forms on our Website or by entering into a contract with us, or through correspondence with us by email, phone or other methods. This includes information provided at the time of registering to use the Website, subscribing to our services and any additional data, posting material, contributions or requesting further services. We may also ask you for information when you report a problem with our Website
· Information we collect about you regarding your use of the website which includes but is not limited to technical information such as IP address, log-in information, browser type/version, device information, URLs, products viewed and searched, site response times, route to our site from other websites etc.
Information we may receive from third parties such as your General Practitioner, and which may include:
· Personal identifiers like your name, address, date of birth, and marital status,
your contact information, including telephone number, home address, and email,
· medical information relevant to the referral, such as your medical history necessary to explain the reason for your consultation,
· the browser and device you used.
We may combine third-party information with information you provided to us/and or information collected by us, which may be used for the purposes we have defined above
If you supply personal data on behalf of someone else, you confirm that the other person has appointed you to act on their behalf and has given you consent to do so.
If you contact us by any written means, we will keep a record of that correspondence as required by Law.
We may also ask you to complete surveys that we use for research purposes. If you do complete a survey the results may be retained, however they are anonymised.
Our Website may, from time to time, contain links to and from the websites of our partner networks, advertisers, and affiliates. If you follow a link to any of these websites, please note that these websites have their own privacy policies and that we do not accept any responsibility or liability for these policies. Please check these policies before you submit any personal data to these websites.
Data Retention
We are required to retain your personal information for as long as we have a relationship with you plus an additional 10 years.
The only exceptions to the periods mentioned above are where:
· The governing law requires us to hold your Personal Data for a longer period or delete it sooner.
· You exercise your right to have the Personal Data erased (where it applies) and we do not need to hold it in connection with any of the reasons permitted or required under governing law.
Data Access
You have the right, at any time, to access, correct, delete or transfer your personal information or to restrict or object to our use of it. If you would like to discuss or exercise these rights, please contact us. We may need extra information from you when dealing with a data request.
You may request a copy of your data at any time. Please provide the following information: your name, telephone number, email address and details of the information you require. We will need to verify your identity so we may ask for a copy of your passport or driving licence and/or recent utility bill.
If you believe any of your personal data we hold on you is inaccurate or incomplete, please contact the us directly and any necessary corrections to your data will be made promptly.

If your personal data that we control is stolen or otherwise breached where this constitutes a high risk to your rights and freedoms, we will contact you without delay. The nature of the breach, the steps we are taking to deal with it and a point of contact for further information will be explained to you.
